Welcome to Burlington!
We are excited to have you join us! The following documents are required of all new employees. Many can be completed online before printing. Print each and return at your earliest convenience.
When you meet with us to go over these documents, please provide the following items:
- 2 forms of identification (as listed on the I-9 form; your choice!)
- Any other licenses required for your new position
- Voided check so we can set up your Direct Deposit
Important Notes
- All forms listed must be completed as thoroughly as possible
- If you have any questions, please feel free to call our office at 781-505-1160
- Incomplete or forms not submitted timely may result in incorrect processing of your first paycheck
- Please date and sign the form where prompted

Permanent Employees:

Payroll/Tax Forms:
- Employee Withholding Allowance Form (W-4) (PDF)
Only page 1 is necessary to complete and return. The only two steps required for all employees are Step 1 (personal information) and Step 5 (signature). Complete Steps 2-4 only if they apply to you. Doing so will make your withholding more accurately match your liability. This online calculator from the Internal Revenue Service is available to help you determine your withholding. - Massachusetts Withholding Certificate (M-4) (PDF)
Complete the top fully. Item Number 4 is required; all others are optional. - Middlesex County Retirement System Enrollment Form (PDF)
Complete all pages as fully as possible. If you are currently a member of the Middlesex County Retirement System, your funds will transfer automatically to your employment in Burlington. If you have participated in other Massachusetts public retirement systems, contact Middlesex Retirement to have those funds transferred into your Middlesex account. Social security numbers are required for all beneficiaries. Please refer to the Middlesex Retirement Beneficiary Guide in the Resources section. - Direct Deposit Form (XLS)
This form is required and allows your paychecks to be deposited into your financial institution(s) and must be returned with a voided check or proof of account. The Burlington Municipal Employees Federal Credit Union is available to all employees if you need to set up a checking/savings account. - Employment Eligibility Verification (I-9)
You will need Adobe Reader to view this fillable document.
Only page 1 is necessary to complete and return along with the required identification. Review the lists of acceptable identification and work eligibility documents on page 3. Return the form along with copies of the required identification and work eligibility documents. If you need assistance in completing this form, instructions can be found in the Resources section.

Employees Working Fewer Than 20 Hours Weekly / Temporary Employees / Seasonal Employees

Only page 1 is necessary to complete and return. As a part-time, seasonal, or temporary employee of a Massachusetts local government employer, you are required to participate in OBRA, which is an alternative to Social Security. Complete this form as fully as possible. Social security numbers are required for all beneficiaries. Please refer to the OBRA information sheet on page 2 of this document. - Statement Concerning Social Security (SSA-1945) (PDF)
Because you are enrolling in OBRA, the Town of Burlington does not deduct social security from your earnings. This form explains how your Social Security benefits are affected. - Direct Deposit Form (XLS)
